1. પ્રસ્તાવના

ભારતનો આરોગ્ય વ્યવસ્થાપન (Health System) વિશ્વના સૌથી વિશાળ અને જટિલ હેલ્થ સિસ્ટમમાંથી એક છે. એક તરફ 140 કરોડથી વધુ વસ્તી, બીજી તરફ આરોગ્યના વિવિધ સામાજિક, આર્થિક અને પર્યાવરણીય નક્કી કારકો (Social Determinants of Health) — આ બધાને સંભાળવા માટે ભારતે વર્ષોથી સુવ્યવસ્થિત, બહુમંજિલી અને તબક્કાવાર હેલ્થ ડિલિવરી સિસ્ટમ વિકસાવી છે.

ભારતનો હેલ્થ સિસ્ટમ મુખ્યત્વે નીચેના ચાર સ્તરો પર નિર્મિત છે:

Sub-Centre (SC) – પ્રાથમિક આરોગ્ય સેવાનો પહેલો સંપર્ક બિંદુ
Primary Health Centre (PHC) – મૂળભૂત અને વ્યાપક પ્રાથમિક સારવાર કેન્દ્ર
Community Health Centre (CHC) – મુખ્યત્વે રેફરલ યુનિટ
District Hospital (DH) – દ્વિતીય/તૃતિય સ્તરની તબીબી સેવાઓ

ભારત સરકારે ગ્રામ્ય અને શહેરી બંને વિસ્તાર માટે અલગ–અલગ આરોગ્ય નીતિઓ, મિશન અને માળખાઓ વિકસાવ્યાં છે જેમ કે:

ભારતનો હેલ્થ સિસ્ટમ આંતરરાષ્ટ્રીય સ્તરે સ્વીકારેલી Primary–Secondary–Tertiary Model પર આધારિત છે. આ મોડલ આરોગ્ય સેવાઓને તબક્કા પ્રમાણે વિભાગિત કરે છે જેથી સામાન્ય રાહદાર થી લઈને ગંભીર દર્દીઓ સુધી તમામને યોગ્ય અને સમયસર સેવા મળી રહે.

ઉદાહરણ તરીકે, એક ANM ગામમાં ઘર–ઘર જઈને ગર્ભાવસ્થા દરમિયાન તપાસ કરે છે, TT રસી આપે છે અને 9 મહિને પૌષ્ટિકતા અંગે માર્ગદર્શન આપે છે. આ રીતે Sub-Centre “Health for All” ના સૂત્રનું પ્રથમ તબક્કું છે.

1. प्रस्तावना

1. Introduction

India’s health management (Health System) is one of the largest and most complex health systems in the world. On one hand, India has a population of more than 1.4 billion, and on the other hand, there are numerous social, economic and environmental determinants of health — to manage all these, India has developed a well-structured, multi-layered and phased health delivery system over the years.

India’s health system is primarily built on four levels:
